IPN Manual Entry: Register New Exam


Register a new exam by clicking Register New IPN Exam in the Data Management menu of the LCSR  IPN Page:




A form opens with the following fields. 


Field Name

Description

Facility Number

This field is filled in automatically, as determined by the user’s profile.

Registry Exam Number

This field is filled in automatically after the exam is entered.

Patient Information

Enter at least one of the fields below.

If this is an exam for a returning patient, and you don't know the patient's identifiers, you can use the Manage Patients function to look up them up by patient name.

NRDR Patient ID
The NRDR patient ID is created by the system when a patient record is created. If you are entering a new patient, leave this field blank.

Patient SSN

Patient Social Security Number must be in the format NNN-NN-NNNN, where N is a digit; all 9 digits are required.


Other Identification

If neither the Patient SSN nor Medicare Beneficiary ID is provided, then the Other Identification field must be filled in. It must be a code that uniquely identifies the patient within your practice, such as a medical record number.


Old Medicare Beneficiary ID 


New Medicare Beneficiary ID

The Medicare Beneficiary ID is also known as the Medicare Health Insurance Claim Number. Enter this field without any special characters. For example, if a beneficiary ID is 123-34-5678A, enter it as 123345678A.


If you are entering an exam for a new patient, leave NRDR Patient ID blank. Enter at least one identifier, either Social Security Number, "Other ID", Old Medicare Beneficiary ID (MBID) or New MBID. "Other ID" must be an identifier that uniquely identifies a patient at your practice, such as medical record number. 


If you are entering an exam for an existing patient, enter one of the five patient ID fields. 


When you click Enter, a new form will open where you can enter exam information. You can also enter additional information about the patient, such as name and date of birth. If the exam is for an existing patient, the patient information will be pre-populated. Refer to IPN Manual Data Entry: Exam Form for more information.
